M3+ Program Overview
What is the M3+ program? 
M3+ is an innovative disease management membership program for individuals who have recently been diagnosed with diabetes or those who have difficulty maintaining glycemic control. The program offers maximum access to staff by assigning members a dedicated health care coordinator. A wide array of educational opportunities are available to members inlcuding resources and classes at m3plus.org, diabetic medications and testing supplies available for pick-up or convenient home delivery from our pharmacy.*
Why should I join M3+?
The M3+ program does not take the place of your existing medical providers, but offers enhanced care and maximum access to services that help you manage your diabetes. As questions come up related to your diabetes, your health care coordinator will help you navigate your way. Added benefits such as priority appointment scheduling and follow-up phone calls and free phone-in prescription medication renewals are also included in the program. Additionally, all M3+ members will receive a free blood glucose meter and meter down-loading so that tracking your blood sugar level is fast and simple.
How will M3+ help me manage my diabetes?
A wide variety of diabetes management resources are available to M3+ members. Free classes with topics ranging from “Advanced Carbohydrate Counting” to “Traveling With Diabetes” are available. These classes, presented in a small group setting, are designed to help you with every aspect of diabetes self management. A members only web site, www.m3plus.org, will provide additional diabetes management information and resources and an electronic newsletter will be sent out highlighting important aspects of diabetes management.
How does the M3+ program help with my medications and supplies?
Membership in the program ensures that your medications and testing supplies are available for pick-up at our pharmacy after every appointment.* For even greater convenience, home delivery of some medications and testing supplies is also available.
What rewards are included in the M3+ Program?
Membership in the M3+ program entitles you to special savings on many health related services. Based on membership levels, all participants receive a FREE Coranary Calcium Scan, Basal Metobolic Rate Test and/or a 30 minute visit with our Registered Dietitian. Additional rewards such as a members only website, prescription medication discounts and free testing supplies are also available to members at the end of every 12 month membership.
What is the cost to the M3+ program?
There is a minimal membership fee of $10 per month to join the M3+ program with a minimum commitment of 6 months. And for a limited time, you can sign up for 12 months for only $100. That’s like getting 2 months free!
